A biomechanical model of percutaneous distal metatarsal osteotomy: load transmission influencing successful follow-up

چکیده
Introduction Symptomatic hallux valgus is a common clinical problem. Current trends for treatment are toward percutaneous or mini-invasive procedures, among these a mini-invasive distal metatarsal osteotomy (SERI) demonstrated to be effective in correcting the deformity with minimal surgical trauma and satisfactory results [1]. Conflicting with previous clinical studies, a recent paper [2] reported discouraging short-term radiographic results by using a theoretically similar osteotomy and fixation device.

Distal osteotomy of the lateral metatarsals: a series of 72 cases comparing the Weil osteotomy and the DMMO percutaneous osteotomy.
INTRODUCTION A Weil osteotomy with internal fixation can match the preoperative plan by precisely setting the metatarsal length; however 10 to 30% of patients end up experiencing postoperative stiffness. A percutaneous distal metatarsal mini-invasive osteotomy (DMMO) is a purely extra-articular technique; metatarsal length is set automatically upon weight bearing of the foot. The goal of this s...
متن کاملHallux valgus (bunions).
INTRODUCTION: Hallux valgus (bunions) are prominent and often inflamed metatarsal heads and overlying bursae. They are associated with valgus deviation of the great toe which moves towards the second toe. Hallux valgus is found in at least 2% of children aged 9 to 10 years, and almost half of adults, with greater prevalence in women. METHODS AND OUTCOMES: We conducted a systematic review and ai...
متن کاملEarly results of distal metatarsal osteotomy through minimally invasive approach for mild-to-moderate hallux valgus.
A prospective study was performed on 24 patients to evaluate the early results of our experience with minimally invasive distal metatarsal osteotomy to treat mild-to-moderate hallux valgus deformities in adults. After failure of conservative treatment and persistence of footwear-related pain, a percutaneous distal first metatarsal osteotomy was performed, stabilized by a Kirschner wire, followe...
متن کاملChevron osteotomy of lesser metatarsals for intractable plantar callosities.
We performed distal chevron osteotomy of the second, third, or fourth metatarsal for painful plantar callosities in 19 non-rheumatoid patients (16 women, 3 men; 21 feet); their mean age was 59 years (32 to 85). The mean follow-up was four years (2 to 7). The overall results were good in 16 feet, fair in two, and poor in three, with four patients still having painful plantar callosities. There w...
متن کاملA comparison of proximal and distal Chevron osteotomy, both with lateral soft-tissue release, for moderate to severe hallux valgus in patients undergoing simultaneous bilateral correction: a prospective randomised controlled trial.
Moderate to severe hallux valgus is conventionally treated by proximal metatarsal osteotomy. Several recent studies have shown that the indications for distal metatarsal osteotomy with a distal soft-tissue procedure could be extended to include moderate to severe hallux valgus. The purpose of this prospective randomised controlled trial was to compare the outcome of proximal and distal Chevron ...
